Hospital Support

Once a Service User is identified for discharge from the hospital, the hospital staff will initiate contact with Social Services to coordinate the necessary discharge assessment. Upon receiving notification from Social Services, our team will collaborate with the hospital staff and assess the Service User’s care needs, either in the hospital or at their home. Whether the hospital visit was pre-planned or an emergency, our team is fully equipped to carry out the assessment in a timely and efficient manner.

Throughout the assessment process, the Service User’s participation and input are highly valued, as we work together to develop a comprehensive and personalized care plan that meets their unique needs and preferences. Our team is dedicated to ensuring that the Service User receives the highest quality of care and support, and we strive to foster a positive and collaborative relationship with both the Service User and their support network.
